صفحه 1611مقدمهزنان در سنین بالاتری مبتلا به سکته ی قلبی حاد می شوند و فشار خون، دیس لپیدمی، دیابت ملیتوس و مرگ و میر در بیمارستان در زنان بیش از مردان می باشد. هدف این مطالعه، تعیین اختلاف در عوامل پیشگویی کننده ی میزان بقای کوتاه مدت در بیماران مبتلا به انفارکتوس قلبی حاد بر اساس جنس بود.روش هااین مطالعه یک مطالعه ی هم گروهی مبتنی بر بیمارستان بود که طی سال های 1378 تا 1387 تمامی بیماران مبتلا به انفارکتوس حاد قلبی را که در بیمارستان های دولتی و خصوصی شهرستان اصفهان و نجف آباد بستری شدند، در بر گرفت. جهت تعیین عوامل پیشگویی کننده ی بقای 28 روزه ی بیماران از آزمون Cox regression تک متغیره و چند متغیره استفاده شد. میزان بقای بیماران با استفاده از آزمون آماری Kaplan-Meier و Logrank به دست آمد.یافته هاخطر نسبی تطبیق شده ی مرگ در هر دو جنس با افزایش سن بیشتر شد؛ به گونه ای که در گروه های سنی80 سال و بیشتر در مردان 7/12 با فاصله ی اطمینان 95 درصد 3/31-14/5 و در زنان 78/8 با فاصله ی اطمینان 95 درصد 1/63-2/1 بود. همچنین انفارکتوس دیواره ی قدامی در هر دو جنس خطر نسبی بالاتری از نوع تحتانی داشت؛ به طوری که به ترتیب در مردان 3 (فاصله ی اطمینان 95 درصد 11/5-78/1) در برابر 74/1 (فاصله ی اطمینان 95 درصد 3-1) و در زنان 16/2 (فاصله ی اطمینان 95 درصد 4-16/1) در برابر 57/1 (فاصله ی اطمینان 95 درصد 94/2-82/0) بود. همچنین میزان بقا در هر دو جنس در طی دوره ی مطالعه افزایش یافت که این افزایش در زنان بارزتر بود.نتیجه گیریبا توجه به روند صعودی میزان بقا در هر دو جنس در پی رخداد انفارکتوس قلبی حاد، می توان فعالیت های ارتقای بهداشت عمومی و مراقبت های پزشکی را با همین روند ادامه داد. همچنین پزشکان باید خدمات درمانی کارامد تری برای بیمارانی که در خطر بیشتر مرگ و میر هستند، ارایه نمایند.

در بسیاری از مطالعات به ارتباط پلی مرفیسم حذف و جایگزینی (Insertion/Deletion یا I/D) در ژن آنزیم مبدل آنژیوتانسین (ACE) و بیماری های قلبی- عروقی اشاره شده است اما نقش این پلی مرفیسم و انسداد گرافت های وریدی در طولانی مدت بعد از عمل جراحی بای پاس عروق عروق کرونر (CABG) بحث برانگیز است. هدف مطالعه حاضر ارزیابی ارتباط پلی مرفیسم ((I/Dدر ژن ACE و انسداد آترواسکروتیک گرافت های وریدی است.
روش هابیمارانی که حداقل 5 سال از زمان عمل CABG آنها گذشته بود در این مطالعه مقطعی شرکت داده شدند. انسداد گرافت های وریدی به وسیله آنژیوگرافی مورد بررسی قرار گرفت.پلی مورفیسم (I/D) در ژن ACE توسط واکنش زنجیره ای پلی مراز (PCR) مورد ارزیابی قرار گرفت.
یافته هادر مجموع 102 (84 مرد) بیمار در این مطالعه شرکت کردند. توزیع فراوانی ژنوتیپ II، ID، DD مشاهده شده به ترتیب 6/23%، 7/62% و 7/13% بود. بین گروه های ژنوتیپی از لحاظ تعداد گرافت های وریدی با انسداد کامل، دارای انسداد نسبی و فاقد ضایعه انسدادی تفاوت معناداری از لحاظ آماری وجود نداشت (P به ترتیب 6/0، 7/0 و 18/0). اختلاف گروه های ژنوتیپی از نظر تعداد ماه های سپری شده بعد از CABG در آستانه معنی دار شدن قرار داشت (06/0=P) و بیانگر این بود که بیماران در گروه ژنوتیپی II در زمان کوتاهتری، دارای تعداد برابر از گرافت های وریدی با انسداد کامل هستند.
نتیجه گیریعلی رغم این که نتایج مطالعه ما، به عدم ارتباط پلی مرفیسم I/D در ژن ACE و انسداد گرافت های وریدی در طولانی مدت بعد از CABG اشاره دارد ولی احتمالا گروه ژنوتیپی II سرعت انسداد گرافت های وریدی را افزایش می دهد.

صفحه 1631مقدمهامروزه داروهایی مانند اتومیدیت و کتامین به علت عوارض ناخواسته ی کمتر قلبی به طور وسیع در جراحی کاتاراکت مورد استفاده قرار می گیرند. در این مطالعه هدف ما بررسی مقایسه ای آثار تجویز دو گروه داروی متفاوت شامل کتامین-میدازولام (K/M) با اتومیدیت-فنتانیل (E/F) در جراحی کاتاراکت بود.روش هااین مطالعه از نوع کارآزمایی بالینی دوسوکور بود. جمعیت مورد مطالعه بیماران کاندید عمل جراحی کاتاراکت بالای 20 سال مراجعه کننده به بیمارستان فیض در نیم سال اول سال 1390 بودند. بیماران پس از اخذ رضایت کتبی وارد مطالعه شدند و در روز عمل به طور تصادفی به دو گروه E/F و K/M تقسیم شدند. قبل از شروع آرام بخشی Sedation)) و در حین آن فشار خون، ضربان قلب، مدت زمان ریکاوری، رضایت جراح و عوارض ریکاوری (تهوع و استفراغ و درد) ثبت شد.یافته ها94 بیمار کاندید عمل جراحی کاتاراکت به روش Sedation در دو گروه E/F و K/M بررسی شدند. در شروع مطالعه بین دو گروه تفاوت معنی داری در فشار خون سیستولی و دیاستولی و ضربان قلب مشاهده نشد. فشار خون سیستولی در دقیقه ی 20 در گروه E/F 127/25 ± 51/148 و در گروه K/M 00/22 ± 30/149 میلی متر جیوه بود (872/0 = P). میانگین رضایت بیمار در گروه K/M به طور معنی داری از گروه E/F بیشتر بود (001/0 > P)، اما رضایت جراح در دو گروه تفاوت معنی داری نداشت (07/0 = P). میانگین مدت زمان ریکاوری در گروه E/F به طور معنی داری از گروه K/M کمتر بود (001/0 > P). میانگین درجه ی Sedation درد و تهوع تفاوت معنی داری در دو گروه نداشت. با این وجود استفراغ تنها در سه نفر از بیماران گروه E/F اتفاق افتاد. هیپوکسی تنها در یک نفر از گروه K/M و میوکلونوس تنها در یک نفر از بیماران گروه E/F اتفاق افتاد.نتیجه گیریدر مجموع این مطالعه نشان داد که استفاده از اتومیدیت و فنتانیل به اندازه ی میدازولام و کتامین در جراحی کاتاراکت به روش Phacoemulsification موثر است.

صفحه 1639مقدمهرخداد لرز پس از عمل جراحی بسیار شایع است و جلوگیری از آن به خصوص در افراد مسن و بیماران قلبی اهمیت دارد. هدف از این مطالعه، مقایسه ی اثربخشی پتیدین و دگزامتازون و دارونما برای پیشگیری از لرز پس از عمل جراحی بود.روش هااین مطالعه به صورت کارآزمایی بالینی دوسوکور بر روی 120 بیمار کاندید عمل جراحی تحت بیهوشی عمومی در مرکز آموزشی درمانی امام خمینی اردبیل انجام شد. بیماران واجد شرایط به طور تصادفی بلوک بندی شدند و به سه گروه تقسیم شدند. روش القا و حفظ بیهوشی در همه ی بیماران یکسان بود. پس از القای بیهوشی، به یک گروه از بیماران پتیدین، به بیماران گروه دوم دگزامتازون و به بیماران گروه سوم نرمال سالین تزریق گردید. کلیه ی اطلاعات با نرم افزار آماری SPSS و آزمون های 2χ و ANOVA آنالیز شد.یافته هابین سه گروه بیماران از نظر سن، جنس، میانگین مدت زمان عمل و ریکاوری تفاوت معنی داری وجود نداشت. بروز لرز در گروه نرمال سالین 19 مورد (5/47 درصد)، در گروه دگزامتازون 4 مورد (10 درصد) و در گروه پتیدین 15 مورد (5/37 درصد) بود و تفاوت معنی داری بین گروه نرمال سالین با دگزامتازون (001/0 > P) و پتیدین (08/0 = P) وجود داشت. گروه دگزامتازون کمترین میزان لرز را داشت.نتیجه گیریمطالعه ی حاضر نشان داد که اثر دگزامتازون در پیشگیری از لرز بهتر از پتیدین است و با توجه به عدم وجود عوارض همودینامیک و کاهش تهوع و استفراغ بعد از عمل در استفاده از دگزامتازون، می توان آن را به عنوان جایگزین پتیدین به خصوص در بیماران دچار عدم ثبات همودینامیک استفاده کرد.

در برنامه ی پزشک خانواده و نظام ارجاع که از سال 1384در مناطق روستایی به اجرا گذاشته شد، پزشک خانواده موظف به غربالگری بیماری های در اولویت، از جمله دیابت نوع 2 و فشار خون ظرف سه سال اول شروع کار و سپس هر سه سال یک بار می باشد. در این تحقیق با بررسی میزان بیماریابی این دو بیماری، عملکرد پزشکان خانواده را ارزیابی کردیم.
روش هااین تحقیق یک مطالعه ی مقطعی توصیفی- تحلیلی بود که در 1390 به صورت سرشماری از تمام مراکز بهداشتی- درمانی روستایی شهرستان اصفهان انجام شد. با رجوع به پرونده ی بیماران مبتلا به فشار خون و دیابت نوع 2، چک لیستی شامل مشخصات دموگرافیک بیماران و مقدار گلوکز یا فشار خون در زمان تشخیص بیماری تکمیل شد. با مقایسه ی آمار فوق و درصد فراوانی این بیماری ها در استان اصفهان، پیشرفت تیم پزشک خانواده در امر بیماریابی بیماری های دیابت نوع 2 و فشارخون بالا، بررسی شد. داده ها به وسیله ی نرم افزار SPSS نسخه ی 18 و آزمون های آماری 2χ و Student-t مورد تجزیه و تحلیل قرار گرفت.
یافته هاطی این بررسی تعداد 3209 نفر مبتلا به فشار خون و تعداد 1684 نفر مبتلا به دیابت شیرین مشخص گردیدند. تیم های پزشک خانواده تاکنون توانسته اند به ترتیب 5/31 و 55 درصد از بیماران مبتلا به فشار خون و دیابت نوع 2 موجود در مناطق روستایی شهرستان اصفهان را شناسایی نمایند و تحت مراقبت قرار دهند. میزان بیماریابی در دو بیماری مورد مطالعه بعد از اجرای برنامه ی پزشک خانواده نسبت به مدت قبل از آن، تفاوت آماری معنی دار داشت.
نتیجه گیریبه دنبال اجرای طرح بیمه ی همگانی سلامت برای روستاییان و آموزش های پیشگیرانه ی پزشک خانواده و تیم سلامت، عملکرد پزشک خانواده در بیماریابی بیماری های مزمن بهبود یافته است.

صفحه 1662مقدمهعفونت های سیستم تناسلی از شایع ترین مشکلات بیماری های زنان هستند و واژینیت کاندیدایی دومین علت شایع عفونت واژن است که حدود 75 درصد زنان بالغ سالم در طول دوران باروری زندگی خود حداقل یک بار به آن دچار می شوند. مطالعه ی حاضر با هدف شناسایی گونه های مخمرهای جدا شده از واژن در دو گروه زنان سالم و بیمار انجام گردید.روش هااین پژوهش یک مطالعه ی توصیفی- تحلیلی بود که در دو گروه افراد مشکوک به واژینیت کاندیدایی و افراد سالم مراجعه کننده به مطب های خصوصی شهر شیراز انجام گرفت. بعد از انجام معاینات بالینی نمونه گیری از ترشحات واژن افراد جهت انجام کشت، رنگ آمیزی گرم و همچنین تعیین pH انجام شد. تفکیک گونه ها بر اساس آزمایش های کروم آگار و کلامیدوسپور انجام گردید.یافته هادر این مطالعه 42 نفر (35 درصد) کشت قارچی مثبت داشتند. ترشحات واژن به عنوان شایع ترین علامت بالینی در 6/78 درصد افراد بیمار و پس از آن خارش (1/57 درصد)، سوزش (6/47 درصد) و درد ناحیه ی شکم (2/45 درصد) مشاهده گردید. شایع ترین گونه های جدا شده از بیماران شامل C.albicans (1/57 درصد)، C.parapsilosis (3/14 درصد)، C.glabrata (9/11 درصد) و C.krusei (5/9 درصد) بودند. همچنین از 1/7 درصد بیماران 2 گونه ی قارچ و از 21 فرد سالم نیز 5 گونه (8/23 درصد) C.albicans جدا گردید.نتیجه گیریبا توجه به این که تنها یک سوم از مراجعین مشکوک به عفونت از طریق کشت به عنوان عفونت کاندیدایی شناخته شدند و با توجه به عدم دقت تشخیص های بالینی، آزمایشات پاراکلینیکی چون کشت در بیماران مشکوک توصیه می گردد.

لیشمانیوز بیماری عفونی انگلی با انتشار وسیع در مناطق معتدله و گرمسیری است. آنتی موان ها خط اول درمان لیشمانیوز جلدی در ایران و بسیاری از نقاط دنیا می باشند. پیش آگهی بیماری با توجه به گونه عامل بیماری مختلف است و انتخاب دارو با گونه ی عامل بیماری ارتباط دارد. هدف از این مطالعه، تعیین گونه ی عامل لیشمانیوز جلدی و بررسی زمان بهبود کامل ضایعات با توجه به گونه ی عامل بیماری بود.
روش هادر این مطالعه تعیین گونه ی انگل Leishmania جدا شده از بیماران با استفاده از DNA استخراج شده ی حاصل از کشت با روش PCR-RFLP (Polymerase chain reaction–restriction fragment length polymorphism) انجام شد. بیماران توسط پزشک معاینه شدند و روند التیام بالینی آن ها بعد از یک دوره ی درمانی با گلوکانتیم مورد بررسی دقیق قرار گرفت و در پرونده ی بیمار ثبت گردید.
یافته هااز مجموع 20 نمونه ی جداشده، همه ی نمونه ها Leishmania major تشخیص داده شدند. میانگین دوره ی بهبود کامل در مبتلایان به لیشمانیوز جلدی مرطوب 58 روز به دست آمد.
نتیجه گیریمیانگین دوره ی بهبود در بیماران مبتلا به لیشمانیوز جلدی خشک طولانی تر از بیماران لیشمانیوز جلدی مرطوب می باشد.

Page 1611BackgroundCompared to men, women suffer from acute myocardial infarction (AMI) at higher age. In addition, hypertension, dyslipidemia, diabetes mellitus, and in-hospital mortality are more frequent in women. This study aimed to determine gender-based differences in predictive factors of short-term survival in patients with acute myocardial infarction.MethodsIn a cohort study based on hospital records during 1999-2008, all patients with AMI admitted to all public and private hospitals in Isfahan and Najaf Abad (center of Iran) were investigated. To determine predictive factors of short-term (28-day) survival, univariate and multivariate Cox regression analyses were used. Survival rate of patients was calculated using Kaplan-Meier and log rank tests.FindingsOverall, 12815 patients with AMI were included (72.6% male). The adjusted relative risk (RR) of death increased with age. In patients over 80 years of age, RR was 12.7 [95% confidence interval (CI): 5.1-31.3] in men and 8.8 (95% CI: 1.2-63.1) in women. In both genders, RR of anterior wall infarction was greater than inferior wall infarction [3.0 (95% CI: 1.8-5.1) vs. 1.7 (95% CI: 1.0-3.0) in men and 2.2 (95% CI: 1.2-4.0) vs. 1.6 (95% CI: 0.8-2.9) in women]. While survival rate increased in both genders during the study period, this improvement was more apparent in women.ConclusionIncreasing trend of survival rate following an AMI in both genders warrants continuous medical care and public health promotion activities. The relation between angiotensin converting enzyme (ACE) insertion/deletion (I/D) polymorphism and cardiovascular diseases was reported previously but the role of this polymorphism and the occlusion of vein grafts in long-term post coronary artery bypass graft (CABG) surgery still has remained controversial. The aim of the present study was to investigate any probable relationship between ACE I/D polymorphism and the atherosclerotic occlusion of vein grafts.
MethodsPatients who undergone CABG surgery more than five years ago, participated in this cross-sectional study. Occlusion of vein graft was determined by angiography. The ACE I/D polymorphism was detected by polymerase chain reaction (PCR) based restriction analysis.
FindingsA total of 102 patients (84 males) were enrolled to the study. The frequency distribution of DD, ID, and II polymorphisms were 23.6%, 62.7%, and 13.7%, respectively. There were no differences among genotypic groups regarding the number of occluded, diseased, and atherosclerosis-free vein grafts (P = 0.6, 0.7, and 0.18, respectively). Patients with II genotype had the same number of completely occluded vein grafts in a marginally significant shorter time after CABG (P = 0.06) compared with the other groups.
ConclusionAlthough the results of our study indicated no association of ACE I/D polymorphisms and the occlusion of vein grafts long-term post-CABG, ACE II genotype may accelerate the rate of occlusion in vein grafts.

Page 1631BackgroundEtomidate and ketamine are commonly used to induce sedation in cataract surgery due to their favorable hemodynamic side-effect profile. This study was performed to compare the efficacy and safety of etomidate/fentanyl with ketamine/midazolam combinations for inducing sedation in cataract surgery with phacoemulsification technique.MethodsThis study was a double-blind active-control clinical trial. 94 patients scheduled for cataract surgery under sedation, were randomly assigned to receive either etomidate/fentanyl or ketamine/midazolam. Participants’ blood pressure, heart rate, nausea and vomiting frequency, decreased arterial O2 saturation episodes, and recovery duration as well as the patients and surgeon satisfaction were recorded during the study period.FindingsNo statistically significant differences were observed in systolic and diastolic blood pressure and heart rate between groups. Mean systolic blood pressure was 148.5 ± 25.1 in etomidate/fentanyl and 149.3 ± 22.0 in ketamin/midazolam groups (P = 0.87). Similarly, the patients of both groups reported the same levels of pain and nausea, but not vomiting episodes which have only happened in etomidate/fentanyl group. In contrast, patient satisfaction was significantly higher in ketamine/midazolam group (P = 0.001), although surgeon satisfaction were comparable with both combinations (P = 0.07). The members of both groups reached a desired level of sedation in 30 minutes and the between-group differences of sedation scores remained non-significant. However, myoclonus was only experienced by the etomidate/fentanyl group patients.ConclusionOverall, this study demonstrated that the combination of etomidate/fentanyl is as efficient and safe as ketamine/midazolam in cataract surgery, without any significant side effects.Keywords: Cataract extraction, Conscious sedation, Etomidate, Midazolam, Ketamine

Page 1639BackgroundPostoperative shivering is very common and could be followed by many problems such as increasing in oxygen consumption, blood pressure, intracranial and intraocular pressure, and postoperative pain. Therefore, the prevention of shivering is important especially in the elderly and ischemic heart disease patients. The goal of this study was to compare the efficacy of pethidine (meperidine), dexamethasone, and placebo in the prevention of postoperative shivering.MethodsThis double-blind placebo-controlled clinical trial was carried out on 120 patients who were candidates for surgery under general anesthesia. The patients were randomly divided into three groups including those who received placebo (a), dexamethasone (b), and pethidine (c). Induction and maintenance of anesthesia for all patients were similar. Central and peripheral temperatures of patients were measured every 5-minute. After induction, normal saline, dexamethasone, and pethidine were injected to groups a, b, and c, respectively. In recovery phase, all patients were monitored for visible shivering.FindingsThere was no significant difference between the three groups regarding gender, age, duration of surgery, and recovery time. Nineteen cases (47.5%) in group (a) had postoperative shivering compared with the 4 cases (10%) in group (b) and the difference between 2 groups was significant (P = 0.001). Similarly, in group (c), 15 cases (37.5%) had shivering that was significantly lower than the placebo group (P = 0.08).ConclusionThe present study showed that pethidine and dexamethasone are effective in preventing postoperative shivering in elective surgeries under general anesthesia. Moreover, dexamethasone is markedly more effective than pethidine. Dexamethasone can be administered after induction of anesthesia as an alternative to pethidine particularly in patients with hemodynamic instability.

Family physician program and referral system is being performed from 2005 in rural areas of Iran. Family physicians are mandated to screen high risk population for high-priority diseases. The aim of the present study is to evaluate the family physicians’ performance in screening of hypertension (HTN) and diabetes mellitus (DM) 5 years after the introduction of family physician program.
MethodsIn a cross-sectional study in 2011 in rural health centers of Isfahan County, we collected the data from health dossier of all hypertensive and diabetic mellitus patients who were cared in these centers. Demographic data, the date of definite diagnosis, the last fasting blood sugar test and blood pressure were registered in a checklist.
FindingsFamily physician teams diagnosed 3209 hypertensive and 1684 diabetic mellitus patients. Based on the prevalence of hypertension and diabetes mellitus in the studied areas, family physicians diagnosed 31.5% of hypertensive patients and 55% of diabetic mellitus patients.
ConclusionIn rural areas which family physician program has been introduced, HTN and DM screening were improved.

Page 1662BackgroundVaginal candidiasis is the second cause of vaginal infections and almost 75% of healthy females get infected by these yeasts at least once during their reproductive age. The present study aimed to isolate and identify Candida species from the patients with vaginal candidiasis and healthy individuals.MethodsThe study was cross-sectional and conducted on patients with suspected vaginal candidiasis referred to the private clinics of Shiraz, Iran and healthy individuals. Following the clinical examination, samples were taken from the vaginal discharges for culturing, gram staining, and determining the pH. Afterwards, the species were differentiated by CHROMagar Candida medium and the formation of chlamydospore.FindingsIn the present study, the culture of the specimens of 42 patients (35%) yielded positive results. Vaginal discharge was the most prevalent clinical symptom of candida vaginitis with a rate of 78.6% followed by itching (57.1%), burning sensation (47.6%), and lower abdominal pain (45.2%). The most frequently identified species was Candida albicans (57.1%), followed by Candida parapsilosis (14.3%), Candida glabrata (11.9%), and Candida krusei (9.5%). Mixed colonization with two yeast species was observed in 7.1% (n=3) of the cases. Of the 21 healthy controls, the culture of the vaginal swabs yielded yeast colonies identified to be Candida albicans in 5 individuals (23.8%).ConclusionSince only one third of the suspected patients were diagnosed as vaginal candidiasis through laboratory tests and also due to the lack of specificity of the symptoms, performing laboratory tests for definite diagnosis of this infection is recommended.Keywords: Candida, Vulvovaginal candidiasis, Signs, Symptoms

Leishmaniasis is a parasitic infectious disease with a wide spectrum in tropical and subtropical areas. Antimoniates are compounds of choice to treat cutaneous leishmaniasis in Iran and many other countries of the world. There are many fluctuations in response to leishmanicidal agents according to type of the disease, involved species of the parasite, and drug regimen.
MethodsThis study was conducted in the Skin Disease and Leishmaniasis Research Center, Isfahan, Iran. Novy-MacNeal-Nicolle (NNN) and Roswell Park Memorial Institute (RPMI) 1640 media were used to isolate parasites from skin lesions of patients. Deoxyribonucleic acid (DNA) extraction of Leishmania isolates was performed and strain identification was determined using specific internal transcribed spacers-1 (ITS-1) primers. After amplification, the resulted products were analyzed using polymerase chain reaction- restriction fragment length polymorphism (PCR-RFLP). Patients were under traditional glucantime therapy and were followed up for 14 weeks to detect their treatment courses.
FindingsPCR-RELP showed that all etiologic agents of cutaneous leishmaniasis (CL) among referral patients in Isfahan were L. major. The mean duration of ulcer recovery of cases with CL was 58 days.
ConclusionThe mean recovery period of patients with zoonotic CL was noticeably shorter than cases with anthroponotic CL.
